Do you currently see Dr. Broyles? Have you recently been referred? Curious about her care at Qliance? Dr. Broyles now offers two types of care for patients, Comprehensive Primary Care and Endocrine Only Care.
To continue your care with Dr. Broyles, there are two options from which to choose:
Option A - Qliance Primary Care, Level 2 - Comprehensive Primary Care
Qliance Primary Care offers unrestricted primary and preventive care for a low monthly care fee in lieu of insurance. At Qliance you will receive personalized care, access to a provider 7 days a week, unhurried 30 to 60 minute office visits and same or next-day appointments for urgent care needs. Level 2 primary care patients have the added benefit of on-site hospital rounds at Swedish Hospital by Dr. Broyles as needed.
Dr. Broyles Monthly Membership Basics
- Unrestricted office visits (no co-pays)
- Primary AND Endocrine care
- Email and phone visits
- On-site hospital care coordination
- Do not bill insurance
Who is this option good for?
- Treatment of endocrine and primary/internal medicine issues
- If you require endocrine and primary care frequently
- Treatment of minor urgent care
Option B – Qliance Endocrine Care, Fee-per-Visit
If you wish, you may consult Dr. Broyles on a fee-per-visit basis via Qliance Endocrine Care for endocrine related issues only. Fee-per-visit consultations will be booked in 30 minutes increments and may be scheduled at your convenience. Though we do not bill any insurance, we will provide you with a statement of service which you may, individually, submit to your insurance company for reimbursement*. Payment is due at the time of service. Please refer to the fee schedule below for specific pricing information.
Dr. Broyles Fee-per-Visit Basics
- Pay for endocrine services as you need them
- Documentation provided for individual insurance reimbursement
- Uninterrupted 30-60 minute office visits
- Presciptions and refills
Who is this option good for?
- Treatment of endocrine issues only
- If you require endocrine care infrequently
* Please check with your insurance company to determine if your carrier and your specific plan allow for individual claim submission and reimbursement.
