Clearing the Way to Well Being

To become a Qliance patient, you pay a one-time, $99 registration fee. After you are registered, you pay an affordable monthly care fee based upon your age. For families, the registration fee is only charged once per billing account (not per person on account). Child rate applies to children (ages 0-19) of adult Qliance patients.

Patient Care Options

To join Qliance, you will need to fill out our Registration Form. On this form, you will be given the opportunity to join the practice and choose a provider as either as a Qliance Level 1 or Qliance Level 2 patient. Both levels of care receive the services outlined in the Patient Services Guide. However, there are four key differences between the service offerings that are outlined below.

Incidental Fees

Your monthly care fee covers the primary and preventive care services described in the Patient Services Guide. However, at times, your care may require durable medical supplies or third-party services that are not covered. You may be asked to pay additional fees for items purchased on-site such as prescription medications and vaccines, and outside services such laboratory tests and x-ray interpretation if needed. If you have health insurance, you may choose to have your laboratory tests billed to your carrier as well as have prescriptions written to a pharmacy that accepts your plan. In all cases, incidental items purchased on-site are provided at or near our cost and prices and relevance to your care are fully discussed with you in advance of being incurred. Membership Contract

There is no long-term contract when you become a Qliance patient. Therefore, you can discontinue your membership at any time. However, Qliance' practice model is based on a continuous membership in which we serve as your medical home, providing care, counseling, coordination and referral as needed. If you quit and then decide to re-join at a later date, you will be required to pay a new registration fee. Other terms for re-registering (if applicable) will be discussed with you at the time you cancel your service.

Payment Options

Monthly care fees may be paid for by credit card, debit card or automatic funds transfer from your checking or savings account only. Quarterly and annual payment options are also available.
